Why is this important?

Most people would rather stay at home and take care of themselves instead of going into a nursing home or hospital.  A higher percentage of a home health agency’s patients staying at home after an episode of home health may indicate that the home health agency has done its job of checking on their patient’s health status during each visit to find and treat problems early.

VNA Home Health’s goal is to keep patients in their home.  Social workers help patients utilize available community resources, make referrals to physical and occupational therapy for increased independence in activities of daily living and teach patients and their families how to manage their disease process at home.
