Why is this important?

An important goal of any home care patient recovering from illness or surgery is to improve their ability to safely walk and move around.

How WellSpan Compares

Last year, WellSpan’s VNA Home Health provided care in the home to 5,235 patients.  The following graph shows the percentage of those patients (for whom improved mobility was a goal) who demonstrated improvement in safely walking and moving around during their period of treatment.

What we are doing to improve our performance.

A workgroup of VNA staff meets regularly to review important information and strategies for improving mobility for their home care patients.  Currently, staff members design an individual care plan for each patient.  This plan assesses the patient’s mobility goals and outlines important steps for improvement, including patient education.
